I will provide the following three documents: • A. Australian or foreign passport or Australian driver’s licence AND • B. Full birth certificate or citizenship certificate AND • C. Medicare card, or Centrelink card, or foreign government issued identity documents.
If your name is shown differently on various documents lodged with your application then you must provide change of name documents evidencing that you are one and the same person, eg Marriage Certificate, Decree Nisi, Change of Name Certificate
The Board will assess whether you meet identity requirements when your application is processed, and may request further documentation.
